Our scheduling coordinator will contact you to confirm your appointment. If you are a new patient please note that we will send you intake forms (Consent for treatment, medical history, etc.) to the email that you provide. Completing this paperwork prior to your scheduled appointment time will allow us to have your appointment run smooth and timely.
